Report on the Current Trend of Commercial Enteral/Parenteral Nutrition in Outpatient

Abstract
Background : This study aimed to investigate the current practices and characteristics of commercial Enteral/Parenteral nutrition (EN/PN) support in outpatients in a tertiary hospital.

Methods : This retrospective study was examined outpatients who were prescribed commercial EN/PN at Samsung Medical Center in 2021. Demographics, disease classification, prescription departments, EN/PN calories, and home artificial nutrition (HAN) consultation were examined.

Results : A total of 1,108 outpatients were enrolled, of which 1,009 were adults (91.1%). The mean age, height, and weight were 58.2¡¾21.8 years, 159.2¡¾18.2 cm, and 53.6¡¾14.5 kg respectively. Two patients were prescribed both EN and PN and we considered them respectively.
Therefore, among the 1,110 outpatients, 1,073 (96.7%) were prescribed EN of which 92.0% were adults, and 37 (3.3%) were prescribed PN of which 59.5% were adults.
In terms of the number of bags, 367,194 (99.0%) bags were EN of which 87.8% were adults¡¯ and 3,622 (1.0%) bags were PN of which 79.7% were pediatrics¡¯.
Regarding disease classification, Neoplasms represented the largest of outpatients who received EN (57.8%), followed by Diseases of the digestive system (11.6%) and Nervous system disease (7.1%). In case of PN, Disease of the digestive system represented the highest percentage at 25.4%, followed by Neoplasms (20.3%) and Congenital malformations (13.6%).

Conclusion : Our findings revealed that the majority of outpatients were prescribed EN, and Neoplasms were the primary indication for EN. However, most of the prescribed EN did not meet the daily calorie requirements and the rate of HAN requests were also found to be very low. Therefore, it is necessary to conduct systematic management by the nutrition support team and active HAN consultation for EN/PN use in outpatients.
